Requirements: Minimum age requirement is 18. Schedule a cleaning and receive your first treatment of ‘Whitening for Life’ and pay a one-time fee of $199. To continue treatments, the patient must schedule an appointment/cleaning every 6 months, and every 6-month visit they get 2 “free” syringes for maintenance.
