This notice is based on the model notice published by the U.S. Department of Health and Human Services.
Your Rights
You have the right to:
- Get a copy of your dental record. Ask us in writing. We will provide a copy or a summary, usually within 30 days, and may charge a reasonable cost-based fee.
- Ask us to correct your record. If you think information is wrong or incomplete, ask us in writing. We may say no, but we will tell you why within 60 days.
- Request confidential communications. You can ask us to contact you at a specific phone number or address, for example only by mail or only at work. We will say yes to all reasonable requests.
- Ask us to limit what we use or share. You can ask us not to use or share certain information for treatment, payment or operations. We are not required to agree, and may say no if it would affect your care. If you pay for a service in full out of pocket, you can ask us not to share that information with your insurer, and we will agree unless the law requires us to share it.
- Get a list of those with whom we have shared information. You can ask for an accounting of disclosures for the six years before the date you ask. It will not include disclosures for treatment, payment, operations or ones you asked us to make. One list per year is free.
- Get a copy of this notice. Ask at any time, even if you agreed to receive it electronically.
- Choose someone to act for you. A person with medical power of attorney or a legal guardian can exercise your rights and make choices about your information. We will confirm their authority before acting.
- File a complaint if you feel your rights are violated. Contact our office using the information at the end of this notice, or file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights at 200 Independence Avenue SW, Washington, DC 20201, by calling 1-877-696-6775, or online at hhs.gov/ocr/privacy/hipaa/complaints. - Sharing information in a disaster relief situation.
- Including your information in a patient directory, if we keep one.
If you cannot tell us your preference, for example if you are unconscious, we may share information if we believe it is in your best interest, or to lessen a serious and imminent threat to health or safety.
We will never share your information for marketing purposes or sell your information without your written permission. We do not share psychotherapy notes, and we do not use your information for fundraising.
